Teleform (developed by Cardiff Software) is one of a category of applications that perform three tasks: (1) create machine-readable data forms; (2) create data bases to contain the data collected using these forms, and (3) read data from forms created and store that data in the previously created data bases.

There are two related categories of technologies:

• The first is on-line surveys, in which a questionnaire is e-mailed to a respondent, who upon answering the questions submits the form, causing the answers to be stored in a data base, which is then available for analysis or for other purposes.
• The other is optical mark reading (OMR).
